Cost of examination is $30.00.  Contact Chief Deputy Steve Maitlen at the Union County Sheriff’s Office at 641-782-7717, Monday thru Friday from 7:00 AM to 3:00 PM to set up an examination.

Affidavit of Salvage Vehicle Repair Forms are available along with a Permit to Drive on the Internet at
http://www.dot.state.ia.us/mvd/omve/salvage.htm .  Customers can print the form themselves and make the signature certifying the vehicle is under their liability to operate to an inspection site location and to its return to their residence.
